Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

Essential Duties:

• Promotes a positive customer service culture which encourages quality patient outcomes and promotes staff retention

• Reviews assessed case management needs as expressed by clients and as indicated on the client treatment plan

• Responsible for a minimum of 30 hours of production per week

• Assists with monitoring patient engagement and communicates with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement

• Prompt scheduling and completion of an initial Mental Health Assessment for all mental health clients (i.e. LOCUS, Columbia Impairment Scare, DECA-C, etc.)

• Create an Admission Note summarizing the client's information (i.e., identifying information, presenting problem, history of present problem, safety precautions, medical needs, etc.)

• Create an individualized treatment plan in conjunction with the client and/or their guardian (in the event they have a court-appointed guardian or are a minor) addressing the needs identified in the assessment

• Create a care and discharge plan for each client according to their needs

• Update and modify the client's individualized treatment throughout treatment

• Conduct individual, group, couples/family counseling as driven by the needs identified in the assessment process and as indicated in the individualized treatment plan

• Create progress notes summarizing therapy session (i.e., goals and objectives addressed, clinical interventions, client response to interventions, and continued therapy plan)

• Create and utilize linkage of community resources for support needs of clients

• Utilize crisis intervention skills/techniques when indicated (including immediate coordination of indicated service given nature of crisis addressed)

• Other duties as assigned

Schedule: Monday - Friday (7:00a - 3:30p) with Saturday rotation; flexibility to accommodate agency's needs

Education and/or Experience: Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred. 

Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) certification or proven experience with substance use treatment  preferred; State of Illinois licensure (LPC, LSW, LCSW, or LCPC) preferred
