Position Overview:
As a Home Health Licensed Practical Nurse (LPN), you will play a crucial role in delivering personalized and holistic care to patients within the comfort of their homes. Working as part of a multidisciplinary team, you will assess patient needs, administer medication, implement care plans, and provide support to enhance overall well-being.

1. Patient Assessment:
   • Conduct thorough patient assessments, including physical and psychosocial evaluations, to develop individualized care plans.
2. Medication Administration:
   • Administer prescribed medications and treatments according to physician orders and established protocols.
3. Wound Care:
   • Provide skilled nursing care, including wound care, dressing changes, and monitoring of vital signs.
4. Patient Education:
   • Educate patients and their families on health conditions, treatment plans, and self-care techniques to promote overall wellness.
5. Care Planning:
   • Collaborate with the interdisciplinary team to develop and update patient care plans based on ongoing assessments and changing needs.
6. Communication:
   • Maintain clear and concise communication with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
7. Documentation:
   • Accurately document patient assessments, interventions, and progress in accordance with regulatory standards and agency policies.
8. Compliance:
   • Adhere to all regulatory requirements, including maintaining confidentiality and safeguarding patient rights.

• Education and Licensure:
  • Graduation from an accredited practical nursing program.
  • Current and valid LPN license.
• Experience:
  • Minimum of 2 years of nursing experience, preferably in home health or a related setting.

  • Excellent communication and interpersonal skills.
  • Proficient in the use of electronic health records (EHR) systems.
• Certifications:
