Provides patient care services that meet the psychosocial, physical, ADL (Activities of Daly Living), and general aspects of care; meets the communication needs of Residents and families; provides care that reflects initiative and responsibility indicative of professional expectations.

Evening shift 4:00 pm - 9:00 pm

Every other weekend expectation for both

Essential Functions

• Providing direct care to Residents, including giving medications and carefully observing and documenting health status.
• Providing effective nursing care to Residents with memory loss or dementia.
• Keeping accurate charts and documentation and monitoring and assisting in coordinating gaps in care.
• Safely transferring Residents.
• Helping to ensure regulatory compliance with respect to nursing practices.
• Providing guidance to PCAs to promote professional nursing practices, to foster quality clinical care and to promote Team Members’ professional development.
• Assisting Residents with dressing, grooming, hygiene/bathing, eating, bathroom tasks, and talking/transportation.
• Responsible for coordinating, tracking annual medical evaluations of Residents.
• Completing all work in strict adherence to Tree of Life Personal Care, LLCs policies and state and federal regulations, including HIPAA and Department of Human Services regulations.

Educational and Other Requirements

1. Graduate of accredited licensed practical/vocational nurse program.

2. Current PA licensure as Licensed Practical/Vocational Nurse.

3. CPR/BCLS certification.

4. Minimum six months’ Medical/Surgical experience in an acute care setting preferred.

5. Excellent oral and written communication and interpersonal skills.

Physical Requirements
