We are seeking a dedicated and compassionate Home Health / Hospice Aide to join our team for the 1st shift, Monday to Friday. As a Home Health / Hospice Aide, you will play a vital role in providing essential care and support to our patients in the comfort of their homes. This position offers an excellent opportunity to make a meaningful impact while working alongside a team of healthcare professionals committed to excellence.

Responsibilities:
  • Ensure the quality and safe delivery of personal hospice services to the hospice patients while also supporting their families and caregivers.
  • Perform or assist patient with personal care activities as prescribed in the care plan, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, reports to the Home Health/Hospice Aide Supervisor.
  • Provide and maintain safe environment for patient, by using equipment safely and properly such as footstools, side rails, oxygen, etc.
  • Practices universal precautions in all contacts with patients.
  • Perform or assist patient/family with home maintenance activities as prescribed in the care plan including, but not limited to light housekeeping and meal preparation.
  • Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship, and providing respite for families.
  • Respect the autonomy and right to confidentiality of all patients and families.
  • Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin condition and basic elements and changes in body functioning and record/reports appropriately.
  • Practice safe transfer, ambulation, normal range of motion and positioning techniques.
  • Identify need for intervention by other members of the interdisciplinary team on an ongoing basis and reports to RN case managers.
  • Document care provided and response of patient/family to care and submit documentation in a timely manner in accordance with agency policies.
  • Assist patient/family/caregiver and other team members in providing continuity of care.
  • Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver. 
Qualifications:
  • High school diploma or equivalent.
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required.
  • Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 prior to working independently with a patient.
  • Previous experience in home health or hospice care is a plus.
  • Compassionate and empathetic attitude towards patients and their families.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Reliable transportation and valid driver's license.
  • Must pass background checks and drug screening.
