Are you a dedicated and compassionate Licensed Practical Nurse (LPN) seeking a fulfilling opportunity to make a difference in the lives of patients and their families? Join our team at the VNA of Hanover & Spring Grove, where we provide exceptional end-of-life care with dignity, respect, and compassion.
Provide skilled nursing care to the hospice patient, family, and caregiver, as directed by the supervising RN, Manager of Support Services and/or Director, in compliance with Medicare Hospice Conditions of Participation, PA licensure laws and Agency policies and procedures. Provide timely documentation of patient services based on the patient’s plan of care. Works closely with other members of the Interdisciplinary Team to ensure all patient and family care needs are being met.
Primary Duties:
  • Ensure the quality and safe delivery of Hospice services to patients/families/caregivers.
  • Implement the patient’s plan of care as directed by the Hospice RN.
  • Provide nursing care by utilizing all elements of nursing process as directed by the hospice plan of care.
  • Responsible for checking and administering medications according to PA Nurse Practice Act.
  • Assess and provide patient/family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participate in coordination of Hospice services, appropriately communicating and reporting the need for Hospice interdisciplinary team member intervention to the Hospice RN, Director or Manager of Support Services.
  • Provide appropriate pain/symptom management and evaluate patient’s response to treatments/medications.
  • Submit accurate and relevant clinical notes regarding patient’s condition and care given.
  • Observe patient for changes in condition and report changes and patient’s, family’s and/or caregiver’s needs to Hospice RN, Director or Manager of Support Services.
  • Assist patient with activities of daily living (e.g., reposition or move patients, bathe, feed. etc.) as required and support patient’s efforts toward self-sufficiency and optimal comfort care.
  • Assist patient/family/caregiver in self-care techniques as appropriate, working in conjunction with other hospice interdisciplinary team members.
  • Provide and maintain a safe environment for the patient.
  • Assist patient/family/caregiver and other team members in providing continuity of care.
  • Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.
Qualifications:
Education:
  • Graduate of an accredited practical nursing program.
Experience:
  • Has minimum of one-year general nursing experience. Community health/hospice or medical/surgical experience is preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy and needs of terminally ill.
Licensure, Registration, and/or Certification:
  • LPN licensed in the Commonwealth of PA.
  • Valid driver’s license and reliable transportation that is insured in accordance with PA requirements.
