True Promise Home Health is looking for a Registered Nurse to add to our home health team in the West Valley. We are dedicated to providing amazing home health care to ALL patients and need YOU on your team!

- Great autonomy working mostly in patients' homes, your personal home for documentation/office work, and occasionally stopping by the agency for the routine training and supplies
- Work directly with patients to achieve maximum medical recovery
- Provide planning and delivery of direct and indirect patient care through the nursing process
- Develop nursing care plan in coordination with patient, family, and interdisciplinary staff as necessary
- Communicate changes in patient's clinical condition with Physicians, Nursing Supervisor, and co-workers
- Record and document patient care services on EMR software
Job Type: Part-time
Pay: From $400.00 per week
Expected hours: 20 – 25 per week
Schedule:
- Monday to Friday
- Weekends as needed
Experience:
- Nursing: 1 year (Required)
- Home health: 2 years (Preferred)
- Case management: 1 year (Preferred)
Work Location: On the road
