Summary of Position Responsibilities
As a Case Manager, you are responsible for helping those with chronic illness, mental health and or substance use disorder or life-altering conditions to gain access to rehabilitation programs and other community services. Your duties include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed. You will perform several tasks to achieve the best outcomes for our clients.

Essential Duties and Responsibilities:
? Case manager must attend 8 team meetings per year.
? Case manager will be responsible for attending MANDATORY ACT Meeting.

Work is usually performed in an office environment; includes continuous contact with staff and the public. Work is performed in an office or other environmentally controlled room; work exposes incumbent to contagious or infectious diseases; work may expose incumbent to dangerous and volatile situations; which could result in bodily injury.

Typical Physical Requirements
Travel within the Baltimore community for majority as needed. Sit for extended periods; frequently stand and walk; normal manual dexterity and eye hand coordination; corrected hearing and vision to normal range; verbal communication; use of office equipment including computers, telephones, calculators, and copiers. Move and lift light objects up to 25 pounds such as mail, supplies, files, and equipment.
