What you will do:
The Lead Case Manager is responsible for supervision and oversight of the case management department and serves as an expert clinical practitioner. As a team leader, the Lead Case Manager provides daily management of personnel issues and staffing. Under the direction of the Manager, Case Management serves as a coach and advisor to staff, a resource, a preceptor to new employees, a reviewer of clinical quality through audits, and follows through on quality improvement initiatives utilizing lean methodology. The Lead Case Manager is also responsible for implementation of the plan of care for specific patients, beginning on a pre-admission basis and following through placement and into follow-up/after care and is empowered to facilitate mid-course care changes, in conjunction with the patient care team.
