- Weekly SUD/Case management meetings on Mondays 12:30-1:30
Complete all initial case management intakes including, case management screening form, family assessment, health risk screening, Columbia suicide screening, and case management goals.
Go over all phases/rules with initial clients and have them sign everything.
Manages all trackers for each phase to ensure clients are meeting case management goals at each level
Oversee all medical and clinical appointments for client
Submits monthly proposals for client outings and monthly outings.
Creates and prints out graduation certificates for all clients graduating SUD program.
Sees all clients weekly for 45-minute increments Mon-Thursdays
Accompanies clients to their medical appointments
