Schedule: Monday - Friday (8:30 am – 5:00 pm) 1st and 2nd Wednesday monthly, (8 am-4:30 pm) Must

Summary
The Program Manager of Case Management will provide oversite to case management staff that are responsible for providing services to single adults experiencing homelessness as they transition into permanent housing. Primary responsibilities of the selected candidate include supervising a team by providing oversite, coordination of services, ensuring that staff work with clients to access best fit housing options, implementation of housing stabilization plans, and coordination of multiple services to meet each clients' needs. The Program Manager is also responsible for building and maintaining good working relationships with landlords/management companies/property owners as well as with other homeless service providers and partnering agencies. The Program Manager will also provide assessment and stabilization services to high-profile clients. The Program Manager will work with the rest of the leadership team to oversee the day-to-day operations of the Access center.
Essential Duties & Responsibilities
Under the supervision of the Assistant Director, the Program Manager is responsible for:
1. Assist the management team with day-to-day operations of The Access Center
2. Along with the Director and additional members of the management team, develop program policies, and procedure plans
3. Assist the Assistant Directors by providing onboarding training to all teams
4. With the management team, supervise volunteer activities
5. Provide individual and team supervision, support, and performance evaluations for assigned staff
6. Recruit, hire and evaluate staff and volunteers within the guidelines of The People Concern Personnel Policies and in conjunction with the Assistant Director & Director
7. Encourage and ensure staff adherence to policies and procedures, as outlined by Director and The People Concern
8. Maintain positive, ongoing relationships with other The People Concern programs, and outside agencies including the City of Santa Monica, Police Department, County agencies, other social service agencies, local businesses and other outreach teams
9. Oversees the case management enrollment process, creates orientation presentation material for clients interested in case management and train case managers in facilitating the case management orientation
10. Provide ongoing training to staff to ensure high quality customer service and achievement of contract outcomes/goals
11. Analyzes collected client data to identify program needs, and assists with program design and implementation
12. Ensures year end outcomes are met by evaluating monthly outcomes and submitting data timely and accurately
13. Perform appropriate data and quality assurance checks on client files, ensuring accuracy and contract compliance
14. in conjunction with the Assistant Director and Project Director assist with reports
15. In conjunction with the Assistant Director, assist with new program implementation by conducting team meetings to streamline information and outline program procedures
16. Provide crisis intervention and case management to high-profile clients
17. Develop additional resources
18. Attend all mandatory meetings and training courses.
19. Assist Project Director and Assistant Director in planning of meetings/retreats
20. Special projects and other duties as assigned
Qualifications
1. Minimum Bachelors degree or (3) years experience in a social service setting
2. Minimum of 2 years’ case management experience
3. Demonstrated excellent organizational abilities; detail oriented and proficiency in prioritizing assignments for timely completion of projects
4. Demonstrated knowledge of issues faced by population served (homelessness, mental illness, substance abuse, medical issues, domestic violence, etc.)
5. Knowledge of Homeless Management Integrated System (HMIS), Clarity and Service Point
6. Knowledge of best practices that include motivational interviewing, harm reduction, trauma Informed Care, housing first, and the ability to apply those practices with participants who have multiple barriers
7. Able to work in a high tolerance Harm Reduction model with participants who have multiple barriers
8. Ability to self-motivate, learn quickly, work effectively with a wide range of constituencies and be flexible in a fast-paced environment
9. Experience working in a community-based setting and as part of a team
10. Committed to improving the community through empowerment and dignity
11. Computer literate; able to use computerized database system for client file management and Proficient in Microsoft Applications: Word, Outlook, and Excel
12. Good writing and interpersonal skills
13. Comfortable, able and willing to drive/transport program participants in agency van
14. Current, valid California Driver’s License with an acceptable driving record, reliable vehicle required for necessary transportation between sites
15. Able to provide cleared TB test & pass a criminal background check
Preferred Qualifications:
1. Bilingual
2. Two (2) years’ experience in direct social services to target population, and working knowledge of needed services and access to services
3. Previous supervisory experience
Work Environment:
1. Combination of field and office environment
2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
3. On occasion walk or drive to different local sites
4. Will necessitate working in busy and loud environments
5. Will be exposed to elements like cold, heat, dust, noise and odor
6. May need to bend, stoop, twist and sit throughout the day
