Summit Community Care Clinic is Hiring a Behavioral Health Provider Near Frisco, CO

Position Summary:

The Integrated Behavioral Health Provider will function as an independent clinician who provides behavioral health counseling to children, adults, couples, and families in an integrated care setting. An option to provide children’s and adolescents’ services if desired. The integrated behavioral health prover works closely with primary care providers, dental providers, psychiatry, and health educators in treating behavioral health and substance use conditions to promote health behavior change. As a part of the primary care team, the Integrated Behavioral Health Provider will provide comprehensive diagnostic and behavioral health services including brief assessments, interventions, psycho education, referrals, and consultations for patients in primary care and school-based health clinic settings. Integrated Behavioral Health Providers work from a systems and community perspective to formulate treatment conceptualizations, interventions, and case management goals.

Integrated Behavioral Health Providers will develop treatment plans with their patients and care teams. They will utilize evidence based psychosocial interventions to achieve treatment goals, such as Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Solution Focused, Eye Movement Desensitization Reprocessing (EMDR) and Problem-Solving Therapy. Integrated Behavioral Health providers may engage in several types of therapeutic modalities in this clinical setting, which may include play therapy, individual therapy, couples therapy, group therapy and case management. The Integrated Behavioral Health Provider must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's individual needs.

Essential Functions:

  • Provide quality clinical counseling to adults, couples, children, and families using evidence based psychosocial interventions.
  • Assess, diagnose, and treat a variety of mental health disorders following DSM diagnostic criteria and evidence-based practice guidelines.
  • Provide education, outreach, and case management to populations with psycho, social, emotional, and behavioral health issues.
  • Promote team-based care through engagement with medical providers’ and other clinical staff to facilitate teamwork, improve patient care processes and outcomes.
  • Provide clinical consultation and serve as a resource for colleagues in behavioral health department and broader community.
  • Facilitate treatment groups.
  • Respond to on-site crisis situations and other patient crisis needs according to policy.
  • Attending regularly scheduled staff meetings, in-service meetings, supervision, and client staffing.
  • Participate in program development through quality initiatives and discussion with team members and supervisor.
  • Perform warm referrals with medical and dental clinics as indicated by PHQ results and as requested by treating providers.
  • Conduct outreach to patients around scheduling, response to medication and other case management needs.
  • Work with care team and providers to increase their understanding of mental health issues with patients.
  • Collaborate with necessary entities to provide aftercare planning and treatment following hospitalization or higher level of care.
  • Work with primary care team to treat and manage patients with chronic behavioral, social, and emotional health problems.
  • Assist in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.

Additional Requirements:

  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required trainings as designated by HR and the position supervisor.

  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.

  • Must participate in quality assurance/ improvement activities when applicable.

Qualifications:

Education/Experience:

Must Haves to be Successful:

  • Bilingual Spanish English is required.
  • Master’s level degree or higher in one of the recognized mental health professions (counseling, social work, psychologist) M.A., M.S. in Clinical Counseling (or related area), LSW, LCSW, LPC, Ph.D. or PsyD. Licensed in the state of Colorado preferred, license eligible considered.
  • Registered with the Department of Regulatory Agencies (DORA).
  • Experience in assessment, diagnosis, and treatment of mental health conditions with adult and/or pediatric populations.
  • Experience in delivering evidence-based therapies is preferred (e.g., EMDR, DBT, CBT, etc).
  • Knowledge and experience in evidence-based models of behavioral healthcare delivered in a primary care setting is a plus, and willingness to grow in these areas is required.
  • Academic and/or job experience in legal and ethical decision making, individual and cultural diversity, diagnosis and treatment planning, evidence-based practice and interventions for psychotherapy, theoretical and conceptualization skills, psychopharmacology and behavioral medicine, professional consultation, integrated care, and clinical administration and utilization of electronic health records.
  • Ability to analyze processes and information, identify problems and trends, and develop effective solutions and strategies.
  • Ability to work in a team and to share responsibilities and duties.
  • Ability to resolve interpersonal conflict in a straightforward, empathic, and timely manner.
  • Ability to be friendly and empathic regarding co-workers and patients.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to work autonomously, reason objectively, and exercise independent judgment.
  • Ability to assess, project, and plan for patients’ needs.
  • Ability to document concisely, accurately, and in a timely manner.
  • Ability to handle a variety of duties that may be interrupted or changed by immediate circumstances.
  • Ability to handle stress while working with patients, co-workers, and community agencies.

Nice to Haves:

  • Two years of directly related experience preferred.
  • Knowledge of Microsoft Office suite, database, and spreadsheet software.
  • Well-organized and self-motivated.

Work Environment:

The work environment is in a healthcare setting and can be fast paced and involve working directly with patients and healthcare professionals. The individual may also work in a clerical setting.
