Summit Community Care Clinic is Hiring a SBHC Behavioral Health Provider Near Leadville, CO

Position Summary:

The Behavioral Health Provider acts as an independent clinician who provides behavioral health counseling to children, adults, and families in a school-based health care setting. The behavioral health provider works closely with primary care providers, dental providers, psychiatry, and school systems in treating behavioral health conditions. As a part of the primary care team, the Behavioral Health Provider will provide comprehensive diagnostic and behavioral health services including brief assessments, interventions, psycho-education, referrals, and consultations for patients in school-based health clinic settings. Behavioral Health Providers work from a systems and community perspective to formulate treatment conceptualizations, interventions, and case management goals.

Behavioral Health Providers will develop treatment plans with their patients/family system and care teams and utilize evidence-based psychosocial interventions to achieve treatment goals. Behavioral Health providers may engage in several types of therapeutic modalities in this clinical setting, which may include play therapy, individual therapy, group therapy, and case management The Behavioral Health Provider must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's individual needs.

Behavioral Health Providers who work in School Based Health Centers are hired with the knowledge that their regular hours are commiserated with the academic calendar year. Due to the seasonality of these positions, this position will maintain a yearly contract, renewed annually in commiseration with the academic school year. The contract intends to establish an agreed-upon prorated payment and time off schedule that is reflective of the school schedule in which you work. It is also intended to establish clear expectations for days worked and days not worked throughout the academic school year and summer breaks. This contract will be established upon hire. SCCC and the employees agree that full-time benefits will be maintained during periods of extended leave due to school closure. Documentation by employees and SCCC may be required to maintain benefit eligibility.

Essential Functions:

  • Provide quality clinical counseling to adults, children, and families using evidence-based psychosocial interventions.
  • Assess, diagnose, and treat a variety of mental health disorders following DSM diagnostic criteria and evidence-based practice guidelines.
  • Provide education, outreach, and case management to populations with psycho, social, emotional, and behavioral health issues.
  • Perform warm referrals with medical and dental clinics as indicated by PHQ results and as requested by treating providers.
  • Respond to on-site crisis situations and other patient crisis needs according to policy.
  • Promote team-based care through engagement with medical providers and other clinical staff to facilitate teamwork, and improve patient care processes and outcomes.
  • Conduct outreach to patients around scheduling, response to medication, and other case management needs.
  • Attend regularly scheduled staff meetings, in-service meetings, supervision, and client staffing.
  • Participate in program development through quality initiatives and discussions with team members and supervisor.
  • Work with care teams and providers to increase their understanding of mental health issues with patients.
  • Collaborate with necessary entities to provide aftercare planning and treatment following hospitalization or a higher level of care.
  • Work with a primary care team to treat and manage patients with chronic behavioral, social, and emotional health problems.

Additional Requirements:

  • Complete all required training as designated by HR and the position supervisor.

  • Comply with all applicable Credentialing and Privileging requirements timely.

Education/Experience:

Must Haves to be successful:

  • Master’s level degree or higher in one of the recognized mental health professions (counseling, social work, marriage, and family therapy). M.A., M.S. in Clinical Counseling (or related area), LSW, LCSW, LPC, LMFT. Licensed in the state of Colorado preferred, license eligible considered.
  • Registered with the Department of Regulatory Agencies (DORA).
  • Knowledge in lifespan development as well as experience in assessment, diagnosis, and treatment of mental health conditions in child and adolescent populations.
  • Academic and/or job experience in legal and ethical decision-making, individual and cultural diversity, diagnosis and treatment planning, evidence-based practice and interventions for psychotherapy, theoretical and conceptualization skills, psychopharmacology and behavioral medicine, professional consultation, integrated care, clinical administration, and utilization of electronic health records.
  • Reading, writing, and clearly speaking the English language is required.
  • Read, analyze, and interpret complex documents.
  • Knowledge of word processing software, spreadsheet software, and database software.
  • Ability to work in a team and to share responsibilities and duties.
  • Ability to resolve interpersonal conflict in a straightforward, empathic, and timely manner.
  • Ability to be friendly and empathic regarding co-workers and patients.

Nice to Haves:

  • Licensed in the state of Colorado preferred, license eligible considered.
  • Experience in evidence-based therapies is preferred (e.g., EMDR, DBT, CBT, etc.).
  • Knowledge and experience in psychopharmacology and behavioral health medicine are a plus, and willingness to grow in these areas is required.
  • Two years of directly related experience is preferred.
  • Couples and family therapy experience is a plus.
  • Read, write, and clearly speak the Spanish language preferred.

Supervisory Responsibilities:

No direct supervisory responsibility.

Work Environment:

The work environment is in a healthcare setting and can be fast-paced and involve working directly with patients and healthcare professionals. The environment is primarily a clinical setting, but the individual may also work in a clerical setting.
