Physical Therapist (PT) - Part-time

Seeking a part-time Physical Therapist to join our team in Long Beach, CA!

Responsibilities

1. Assess available resident information and determine its impact on the assessment and treatment process.

2. Perform a comprehensive functional assessment of the resident and document the findings.

3. Develop and execute an individualized treatment plan based on the resident's needs and goals.

4. Adhere to evaluation, treatment, and documentation guidelines and maintain accurate department records.

5. Provide instruction and supervision to Assistants during treatment procedures.

6. Document the progress of each treatment session in the resident's record.

7. Update treatment plans as necessary throughout the treatment process.

8. Summarize the course of treatment in a clear and concise discharge summary.

9. Educate residents about program services, treatment procedures, and potential risks.

10. Collaborate with nursing staff, residents, and families to maximize functional abilities.

11. Instruct residents and caregivers on exercise programs and therapeutic procedures for home continuation.

12. Assist with the referral process for additional services following discharge.

13. Participate in rehabilitation team conferences and other department / facility meetings.

14. Pursue continuing education opportunities to maintain clinical competency.

15. Contribute to the quality improvement process.

16. Promote the programs and services of Heritage Healthcare in the community.

17. Comply with regulatory and professional standards and guidelines.

18. Provide feedback to the Rehab Manager on the performance of Assistants.

19. Adhere to facility policies and procedures, and participate in quality improvement and safety programs.

20. Perform other duties as assigned to ensure quality care for residents.

21. Understand reimbursement guidelines and allocate therapy resources accordingly.

22. Maintain knowledge of regulations and guidelines relevant to the field.

23. Support the company's commitment to Compassionate Care.

24. Work independently and seek supervision when necessary.

25. Meet productivity expectations for the position.

Qualifications

1. Bachelor / Masters of Science degree in Physical Therapy.

2. Current license in the state of practice.

3. Prefer experience in a similar setting serving the same age / type of resident population.

4. Certification in specialized areas, if applicable.

5. Excellent communication and interpersonal skills.

6. Competency in physical therapy principles, methods, materials, and equipment.

7. Ability to assess resident needs and develop comprehensive care plans.

8. Knowledge of accreditation standards and compliance requirements.

9. Ability to perform physical tasks required for the job safely.
