This is a new position designed for Moving Home Minnesota project outreach, engagement and training, and support throughout the state to ensure equitable access to the benefit for all eligible Minnesotan’s. This position will obtain and utilize regular feedback, as well as share program communications while interacting with stakeholders to ensure the project remains relevant and reflective of the current needs in the community. This position will also spend a portion of their time coordinating and supporting the participants, providers, and lead agencies through the MHM eligibility, enrollment, and delivery of home and community-based services through the first year of community living.
This position holds a critical component to the success of the MHM project as MHM becomes more accessible and familiar to lead agencies, institutional settings, and community residential providers. The individual will ensure lead agencies and providers know how to access and integrate the grant/ benefit into their existing human services systems, how to weave the benefit with other HCBS programs and benefits, as well as to support capacity building throughout the state.
The individual in this position will play a pivotal role in the outreach and communications for Moving Home Minnesota, as well as for all transition program services and benefits available in Minnesota. Minnesota’s Moving Home Minnesota (MHM) project is a rebalancing demonstration grant federally known as Money Follows the Person. MHM is a systems transformation grant designed to “rebalance” Minnesota’s Medicaid system and home and community-based services (HCBS). The MFP/MHM project is currently funded through 2031-2032, with the possibility of continued funding or to become a permanent program.
