Reports to: Program Director
Job Title: Director of Behavioral Supports & Person-Centered Planning
Summary:
The Behavioral Director & person-centered services will work closely with Serenity leadership team to assure high quality Residential Supported Living services to Serenity’s resident's complex medical and behavioral needs.
The position will provide:
  • Integration of person-centered planning, instructional support, community inclusion, client financial, and positive behavioral support planning
  • Leadership and supervision of the Positive Behavioral Support and Client Finance
  • team members
  • Functional Assessment and positive behavioral support (PBS) planning, consultation, documentation, and reporting required for State and grant contracts
  • Oversite and consultation for individualized instruction and support planning, documentation, and reporting required for State and grant contracts
  • Oversite of client financials, especially as related to individual service, positive behavioral support, and financial planning. Reviews, tracks, and verifies the client financials, client loans, Residential Allowance Requests, and representative payee status.
  • Coordination of client intake and transition processes
  • The leadership of crisis response for Residential Supported Living clients
  • Development of agency-wide understanding and skills in positive behavioral support, person centered services, diversity, equity, and inclusion, trauma-informed care, and other best practices and values of Residential Supported Living
  • Other duties and projects as assigned.

Primary Responsibilities:
The Behavior Support Director is responsible for assessing our residents and creating Behavior Support Plans. These plans are written document that describes individualized proactive support strategies which are designed to replace challenging behaviors with functional, positive behaviors
and which identifies caregivers’ interventions to help them deescalate, reduce or tolerate the challenging behavior. The strategies must focus on environmental, social, and physical factors that affect the behavior(s). Behavior Support Coordinator is also responsible for the development of a Positive Behavior Support plan and Activity plan; teaching activities for designated caregivers on how to implement the plan; monitoring to evaluate the plans impact and as needed; reassessment and revision of the plan and updated teaching and coaching activities. BSS must be provided at the residence where the eligible person lives.
  • Assist with Screening of referrals for admission as needed
  • Create a behavior assessment which is started at screening and completed 10 business days after admission
  • Create a behavior support plan dedicated for each Individual within 15 days of admission
  • The behavioral support plan must align with the individuals service plan, and must be explained to the resident in a manner that they can understand
  • Be an active member of the service planning team
  • Serve as Liaison to DHS Case Managers
  • Provide Individual-specific coaching and group teaching for direct care staff to ensure that direct care staff can implement the strategies defined in each individual's behavior support plan
  • Partners with the Health Services team on behavioral education, and individualized interventions which will then be communicated through Individual behavior plans to direct care staff
  • Consultation to staff and other partners on mitigating behaviors that may place the individual's health and safety at risk to prevent institutionalization
  • Respond to Individual's needs and issues as needed
  • Monitors and evaluates individual behavioral plans, reassess, and revises the plans as needed while providing updated education
  • Participate in weekly IDG meeting
  • Participate/Facilitate in Stand up and 24-Hour follow through
  • AII other duties as assigned
Job Scope:
Duties are performed under direct supervision of Executive Director and Program Director with established procedures and guidelines.
Specific Skills:
  • Excellent interpersonal skills
  • Experience in a community-based residential program serving individuals with intellectual/developmental disabilities and challenging behaviors
  • Ability to respond to and manage emergency situations and crisis behaviors
  • Ability to work as a team player
  • Ability to maintain cooperative working relationships with those contacted in the course of work activities
  • Communicate effectively in English both verbally and in writing
  • Knowledge of principles and practices of residents needs
  • Skill in problem-solving, and the ability to remain poised in emergency situations
  • Ability to understand and work under a variety of situations in a positive constructive manner
  • Ability to use and teach creativity in meeting residents emotional and physical needs
  • Ability to be organized, prioritize, and set and meet deadlines
  • Ability to do many daily tasks via computer, i.e. emails, spreadsheets, etc.
  • High degree of integrity in all areas of conduct related too or reflecting on the organization.
  • Ability to delegate responsibility while maintaining focus of daily activities
  • Knowledge and understanding of state and federal regulations
Education and/or Experience:
  • Degree in Special Education, Psychology, or Human Services field desired
  • Previous DD or APD group home or facility experience delivering Positive behavior Supports to individuals with complex behavioral needs. requirements preferred
  • At least one or more years of psychology/counseling experience or education preferred
  • Valid Oregon drivers license
Job Conditions:
  • This job will require 40 plus hours per week and a flexibility of hours (i.e. weekends, nights as requested)
  • This job may require day and overnight traveling to seminars, trainings, Resident Assessments, etc.
  • Possess and maintains certification from a Department approved program
  • Maintain compliance with continuing education requirements under 411-046-0210
  • Pass DHS Background Check
