Requirements:

• High school graduate
• Bachelor’s in Social Work or related field (preferred but not required)
• Skilled Nursing experience preferred
• Must have knowledge of State and Federal guidelines
• Have proficient communication and documentation skills
• Able to relay positive feedback to families and residents
• Conduct assessments using social work techniques
• Highly organized with the ability to efficiently manage their time to complete multiple tasks

Responsibilities:

• Conduct psychosocial assessments for new residents and develop individualized care plans
• Provide emotional support to residents and families and assist in adjusting to life changes
• Coordinate with healthcare providers and community resources to ensure residents receive necessary services and support
• Advocate for residents' rights and ensure they receive quality care
• Facilitate group therapy sessions and provide individual counseling as needed
• Maintain accurate and complete documentation of resident progress and care plan updates
• Collaborate with interdisciplinary team members to provide comprehensive care to residents
• Participate in quality improvement initiatives and stay current with best practices in social work and nursing home care
• Provide education and training to staff members on social work and resident care topics as needed.
