San Francisco Community Health Center is Hiring a TACE Program Manager Near San Francisco, CA

The TACE (Tenderloin Area Center of Excellence) program aims to ensure and expand continuous seamless access to quality primary care and critical support services for “severe need” and “special populations” who are severely under-served residents of the Tenderloin. Targeted populations include homeless and marginally-housed residents of the Tenderloin, including residents who are active substance users, veterans, residents coping with mental illness, sex workers, transgender individuals, non-English speaking residents, gay males and their non-gay identified sex partners. The focus of TACE is to enhance HIV prevention services for members of these defined populations already living with HIV/AIDS who demonstrate a need for support in developing skills and behaviors to contain the spread of HIV/AIDS to their sexual and drug using partners.

SFCHC staff provide case management, care navigation, drop-in services, and weekly groups. Tom Waddell Urban Health Center provides on-site psychiatric support and medical services. 

Position Description: 

The TACE Program Manager will provide day-to-day oversight, management, and coordination over the TACE Program. The TACE Program Manager will also be responsible for supervising and supporting Case Managers and Nutritional Services staff.

Essential Functions:

  • Serve as day-to-day contact person for all program staff.
  • Coordinate all aspects of client care, client drop in, art therapy program, behavioral health groups and medical clinic supportive services.
  • Maintain and provide support in daily Drop In space.
  • Maintain weekly Drop-In Clinic and space at SFCHC in collaboration with TWUHC.
  • Ensure that all direct service staff provide strengths-based, trauma-informed, and culturally competent services to clients.
  • Deliver or arrange training and resources required for direct reports and direct service staff to perform their roles.
  • Monitor and ensure program deliverables are being met.
  • Review program objectives and performance and design strategies, as needed, to improve service delivery.
  • Develop and maintain protocols to ensure client engagement in care and attending medical appointments.
  • Develop protocols to ensure seamless integration for program.
  • Assist with development and tracking of client treatment plans.
  • Work with collaborating agencies to ensure strong partnerships are maintained.
  • Develop, maintain, and support schedule of SFCHC staff, including (1) requests for home visits/outreach needs and (2) open access programming.
  • Coordinate, attend, and facilitate case conferences and administration meetings with SFCHC and TWUHC staff.
  • Supervise, evaluate, and hire direct reports.
  • Support supervisees in identifying and reaching professional growth goals.
  • Assist the team with crisis management when there is a client crisis that arises.
  • Conduct outreach in various types of settings.
  • Support and maintain program’s food inventory and art therapy inventory.
  • Provide Nutritional Services support as needed.
  • Assist with other administrative duties as needed.

Minimum Qualifications:

  • Bachelor’s degree in health or related field. A combination of experience and education will be considered.
  • At least 1 year experience managing or supervising a team.
  • At least 2 years experience working with dually diagnosed unhoused/ housed individuals communities living with HIV.
  • At least 2 years experience with advocacy, counseling, referrals, or outreach services in the HIV field
  • Familiarity with issues and concerns of the LGBTQ community, sex workers and substance users; immigrants and refugees. Sensitivity to cultural and ethnic concerns as it relates to substance abuse, mental health and HIV. 
  • Ability to maintain confidentiality and privacy of persons, documents, data, and communications.
  • Ability to handle multiple tasks, work under pressure and to work professionally and ethically within multi-racial and multicultural settings; and appropriately seek out support. 
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills, including attention to and respect for detail, accuracy, deadlines and follow up. 
  • Intermediate Microsoft Office skills.
  • Knowledge of HIV/STD transmission and prevention. 
  • Knowledge of health and social issues related to HIV transmission in high-risk populations.
  • Demonstrated experience facilitating community groups/meetings.
  • Ability to work with and maintain boundaries with clients and confidential information.
  • Strong organization skills and ability to apply new knowledge into day-to-day client work.
  • Self-directed, motivated, and flexible with the ability to work independently and as part of a team. 
  • Ability to work some weekends, and to be punctual and perform tasks consistently during scheduled shifts. 
  • Ability to support the mission and goals of SFCHC.
  • Flexible schedule with ability to work weekends, evenings, and non-traditional work hours. 
  • Must be able to sit, stand, and walk for extended periods of time.
  • Fully vaccinated against COVID-19 and able to show proof of vaccination and booster shot.
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine.
  • Proof of TB clearance (issued within the past 12 months or less).

Preferred Qualifications:

  • Master’s Degree in health-related field (MSW, MPA).
  • Knowledge and experience with the drivers and health disparities in the LGBTQ community and how they impact linkage and retention to HIV care services for HIV LGBTQ individuals.
  • Bilingual and bicultural understanding.
  • 2 year of direct supervision experience.
  • 2 year of Case Management experience.
  • 2 year of experience with curriculum development.
  • Professional experience working with people living with HIV and those dealing with substance use and mental health complexities.
  • Knowledge and experience with the drivers and health disparities in the LGBTQ community and how they impact linkage and retention to HIV care services for HIV LGBTQ individuals.
  • Must be able to lift up to 50lbs.
