San Francisco Community Health Center is Hiring a Program Manager (Trans Services) Near San Francisco, CA

The Program Manager for Trans Services will solely be responsible for the day-to-day management, coordination and overall oversight of the Trans Services department of San Francisco Community Health Center. Trans Services encompasses the following contracts: Trans:Thrive and What’s TEA?! and any additional contracts as needed. 

Trans:Thrive offers a community drop-in space for the trans and GNC community that provides groups/workshops, prevention case management, referrals/linkages, HIV testing and community outreach to improve transgender clients' health and wellbeing, reduce risk for HIV and substance use, and provide referrals/linkages to resources. 

What’s TEA?! serves trans youth and GNC of color, ages 14- 34. What’s TEA?! serves the San Francisco and Oakland areas.

The Trans Services Program Manager will also be responsible for supervising and supporting case managers and program staff. The Program Manager for Trans Services will report to the Associate Director of Trans Services.

Essential Duties:

  • Take lead in the day-to-day operations and serve as contact person and be responsible for management, coordination and overall oversight of the Trans Services programs.
  • Lead, manage, support and supervise a team of program staff of Trans Services around job performance, well-defined goals and delivery and development and implementation of staff & program work plans, staff morale maintenance, problem solving and any program- and staff-related issues.
  • Work collaboratively with Associate Director of Health Equity to support review and implement contract goals, program objectives, program budget and design strategies as needed to improve service delivery and ensure deliverables and deadlines are met in a timely fashion through creation, organization, and implementation of project timelines, calendars and schedules.
  • Oversee and manage data collection, data entry, maintenance, and timely submission of documents for monthly reports and track client enrollment/activities and contractual program documentation and reporting requirements.
  • Hire, manage, evaluate and professionally develop case managers and ensure that all case managers provide caliber case management and advocacy services to clients, engage in follow-up conversations and referrals that are result oriented to additional services as necessary.
  • Be able to assist the team with crisis management when there is a client crisis that arises.
  • Lead the Trans Services on how to best develop, distribute and provide outreach information for the programs & clinic, including but not limited to pamphlets, brochures, ads, website information, and social media posts.
  • Actively participate & engage as a representative of the agency and develop external community partnerships with various trans services that are relevant, strategic, and aligned with mission of the programs & clinic.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Participate in required department, inter-departmental and agency-wide meetings, trainings, and activities, which may include some evening and weekend hours. 
  • Other duties and responsibilities as assigned.

Minimum Qualifications:

  • 1-3 years’ experience managing contracts and staff, implementing programs.
  • 3-5 years of work experience in case management, providing counseling, advocacy and education.
  • Working experience with the Trans & GNC communities.
  • Bachelor's degree or a combination of experience and education will be considered.
  • Familiarity and knowledge with enrollment and eligibility with public benefits and health insurance programs and in San Francisco or California.
  • Experience and comfortable in conducting outreach in various types of settings. 
  • Experience in working with multiply-diagnosed patients (HIV, mental health illness & substance abuse).
  • Familiarity with issues and concerns of the LGBT community, sex workers and substance users; immigrants and refugees. Sensitivity to cultural and ethnic concerns as it relates to substance abuse, mental health and HIV
  • Ability to maintain confidentiality and privacy of persons, documents, data, and communications 
  • Ability to handle multiple tasks, work under pressure and to work professionally and ethically within multi-racial and multicultural settings; and appropriately seek out support.
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills, including attention to and respect for detail, accuracy, deadlines and follow up.
  • Intermediate Microsoft Office skills. 
  • Flexible schedule with ability to work weekends, evenings, and non-traditional work hours
  • Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)

Preferred Qualifications:

  • Master’s degree in health science or social work or related field. 
  • 5 years of work experience in case management, providing counseling, advocacy and education.
  • Experience with public benefits and health insurance programs in San Francisco or California.
  • Familiarity with resources for TGNC folks available in San Francisco or California. 
  • Fluency in Spanish.
