San Francisco Community Health Center is Hiring a BHCoE Case Manager I Near San Francisco, CA

 

The Case Manager Level I is responsible for providing psychosocial assessments, coordinating medical care plans with primary medical and behavioral health providers, health education, supportive counseling, crisis intervention, benefits advocacy, and coordinating services within their program as assigned by their direct supervisor. Direct client work and interaction will be done both in the field and at the office.

About the Program:

The Black Health Center of Excellence (BHCoE) serves Black/African-American individuals living with HIV/AIDS who are accessing care at UCSF’s Ward 86 and MOCP. As a non-medical case management program, SFCHC’s BHCoE initiative aims to support enrolled clients with their medical engagement by providing strategic outreach and care linkage, strengths-based case management, resource navigation, treatment adherence counseling, clinical outreach and navigation, case consultation and coordination, and programmatic outreach and coordination. The BHCoE is part of a larger network of Centers of Excellence developed to support people living with HIV/AIDS and receives its funding through the Ryan White Care Act.

Essential Functions:

· Intake and assessment of new clients

· Case management and advocacy services for clients

· Basic HIV treatment education and advocacy, as well as general health education

· Supportive counseling and developing service goals with clients

· Conduct individual risk reduction counseling sessions

· Provide information and referrals to external services and monitor referral completion

· Work with Care Services Team to identify new clients’ needs and develop goals accordingly

· Navigate and outreach clients as needed to support with care engagement

· Participate in community events to identify potential program/agency participants

· Complete necessary documentation, UOS reports and treatment plans in a timely manner

· Participate in biweekly case conferences, monthly staff meetings, and in-service training

· Participation in and compliance with agency quality assurance, peer review, clinical consultation

· Perform other duties and tasks as needed

Minimum Qualifications:

· Fully vaccinated against COVID-19, complete with booster shot, and able to show proof of vaccination

· BA in a health sciences or social work-related field or AAA/CMH/CHW OR comparable work experience

· One year of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities

· Experience working with diverse populations including BIPOC, LGBTQ communities, refugees/immigrants, people living with HIV / AIDS, people experiencing homelessness and multiple diagnosed populations (substance use and/or behavioral health issues)

· Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.

· Must be adaptive, flexible and a team player

· Strong verbal and written proficiency in English

· Experience working with diverse sexual/gender orientations, the homeless, limited English speakers, and/or multiple-diagnosed individuals

· Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills

· Familiarity and experience with Microsoft Word and Outlook programs

· Detail-oriented with strong follow-through skills, including attention to and respect for detail

· Must be able to sit, stand, and walk for extended periods of time

· Up-to-date with vaccinations, especially seasonal flu shot vaccine

· Proof of TB clearance (issued within the past 12 months or less)
