Samuel U. Rodgers Health Center is Hiring a RN Care Manager Near Kansas, MO

The Nurse Care Manager assumes the role as a professional Nurse Educator. The educator plans, organizes, and directs health education programs for Health Home patients; educates patients/families about chronic disease, including treatment options, lifestyle modification, and potential complications; collaborates with members of the health home team, providers and patients/families to identify health needs and develop measurable health goals; monitors patients’ progress toward goals and reports performance measures and outcomes as required by health home and organization; collaborates with health home team and providers to integrate behavioral health into primary care and to identify behavioral health concerns that present barriers to improving health outcomes; uses motivational interviewing techniques to encourage patients to implement self-management strategies that will improve their chronic health condition; ensures educational strategies adhere to current evidence-based guidelines; ensures chronic disease and preventive measures are up to date and coincide with organizational mission and values and health home initiatives.

Responsibilities:

  • Develop wellness and prevention initiatives
  • Facilitate health education groups; provide training on medical diseases, treatment and medications
  • Participate in treatment plan development for all health home enrollees
  • Collaborate with health home team to integrate behavioral health into primary care
  • Identify and address patient care needs during transitions of care
  • Identify and address barriers to improved self-management skills and health outcomes

Professional

  • Active registered nurse licensure in Missouri
  • Ability to demonstrate expertise in the area of adult education

Education

  • Bachelor of Science in Nursing
  • Certified Educator (preferred, not required)
