SWOPE HEALTH is Hiring a Nurse Care Manager Behavioral Health Near Kansas, MO

JOB SUMMARY:

Swope Health is seeking a Nurse Care Manager for the Behavioral Health Department, which will be a Full time Role. Successful candidate will be an active member of the care team and will manage a full array of physical health needs, behavioral health needs, long-term community care services, and family services for designated high risk patients. The Nurse Care Manager ensures that high risk patients have updated care plans and provides support to primary care providers, community support staff (CSS's), and care coordinators. 

PRIMARY FUNCTIONS:

  1. Assists patient through the healthcare system:
  2. Acting as a patient advocate and navigator
  3. Coordinating care provided throughout the full continuum of care, including but not limited to ambulatory care, acute inpatient care, skilled care, home health care, rehabilitative care, pharmacies, DME companies and community programs.
  4. Maintain communication between Primary care and Behavioral care teams and other outside providers involved with patient care
  5. Encourage improved patient self-management ability by facilitating individualized patient education referrals and a
  6. Coordinates continuity of patient care with providers, patients and families following hospital admission, discharge, and ER visits.
  7. Facilitates and monitors patient medication management, education and adherence and provides intervention when needed to help improve understanding and adherence
  8. Assists with Annual Wellness visits and other preventive care services for patients
  9. Tracks quality and performance measures for all patients enrolled into our ACO
  10. Identifies gaps in care and works to correct such gaps through care coordination, protocols and standing orders.
  11. Identifies barriers to closing care gaps and collaborates with other medical staff, providers, and resources to attempt to resolve barriers
  12. Participates on a team for data collection, health outcomes reporting, clinical audits.
  13. Monitors patient care utilization
  14. Assessing the appropriateness of services rendered
  15. Attempt to reduce unnecessary costly expenditures (ER, hospital, and high cost testing) through the above activities
  16. Make contact with new and established Medicare ACO patients to answer questions, provide information about the clinics and services and perform health risk assessments
  17. Promotes effective working relations as part of a team to facilitate the team's ability to meet its goals and objectives.

SECONDARY FUNCTIONS:

  1. Acts as primary contact point for Care Coordinators on the population health team regarding clinical questions and clarification
  2. Attends all required training and continuing education as required or provided.
  3. Other duties and/or responsibilities as assigned by manager or director

QUALIFICATIONS:

  • Have a active Missouri and Kansas , or equivalent multi-state compact license, to practice nursing as a Licensed Practical Nurse -LPN or Registered Nurse- RN in good standing.
  • 2 to 3 years in a healthcare setting to include inpatient/outpatient medical surgical and psychiatric nursing experience preferred
  • Proficient in Micro Soft office apps
  • Current BLS -CPR certification
