Redwoods Post Acute is a skilled nursing facility with 152 beds located in the heart of Willow Glen in San Jose California.

We are seeking a highly motivated and compassionate individual to join our team as a Social Service Assistant for our skilled nursing facility. RESPONSIBILITIES

• Provide direct support and assistance to residents in various activities of daily living.
• Conduct comprehensive assessments of each resident's physical, emotional, and social needs.
• Develop individualized care plans that address the specific needs of each resident.
• Coordinate with other healthcare professionals to ensure continuity of care.
• Assist residents with social integration and participation in community activities.
• Advocate on behalf of residents to ensure their rights and preferences are respected.
• Maintain accurate and up-to-date documentation of resident interactions and progress.

REQUIREMENTS

• High school diploma or equivalent.
• Knowledge of social service policies, procedures, and regulations.
• Strong communication and interpersonal skills.
• Ability to work well in a team environment.
• Empathy and compassion for the elderly population.
• Excellent organizational and time management skills.

EXPERIENCE:

• With previous experience working in a similar role or a healthcare setting are welcome
• Experience working with older adults, and skilled nursing facility
• Experienced Social Service Assistant
