RECLAMATION CENTER is Hiring a Targeted Case Manager Near Fort Lauderdale, FL
Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Collaborate with other professionals and agencies to coordinate services and resources
- Monitor client progress and make necessary adjustments to service plans
- Advocate for clients and ensure they have access to necessary support and resources
- Provide crisis intervention and support during emergencies or high-stress situations
- Assist clients in navigating the healthcare system, including understanding Medicare benefits
- Offer mentoring and guidance to clients, helping them develop skills for self-sufficiency
- Supervise and support a team of case management staff
- Stay updated on current trends and best practices in social work, behavioral health, child welfare, addiction counseling, and workforce development
Qualifications:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Previous experience in case management or a related field
- Knowledge of Medicare regulations and benefits
- Strong understanding of social work principles and practices
- Ability to provide effective crisis management and intervention
- Excellent communication skills, both written and verbal
- Strong organizational skills and ability to prioritize tasks effectively
- Ability to work collaboratively with diverse populations and professionals from various disciplines

Job Type: Contract
Pay: $17.00 - $24.82 per hour
Expected hours: 35 – 40 per week
