Social Service Assistant Responsibilities:
- Complete initial resident assessments
- Coordinate care and Implement care plans with interdisciplinary team
- Assist with discharge planning
- Coordinate resident psychosocial, mental health and behavioral support
- Document patient progress and coordinate family meetings
- Ensure regulatory compliance
- Complete MDS sections as assigned, ensure all areas are addressed in the care plan as necessary.

Requirements:
- High School Degree or GED
- Prior Healthcare Experience, Social Work or Case Management a plus
- Interpersonal, critical thinking, and organizational skills
