POWER's mission is to help women reclaim their lives from addiction and related emotional health issues and improve the well-being of future generations. We are an agency committed to providing quality gender-responsive, trauma-informed treatment and recovery supportive services to women with substance use and co-occurring disorders.
The Case Manager provides case management to POWER House clients and is responsible for assisting clients with acquiring needed resources focusing on, but not limited to, workforce development for clients (as well as support in the areas of aftercare, physical health, other behavioral health, housing, transportation, childcare and recovery supports).
Essential Duties and Responsibilities
Participate in Client orientations, Treatment Team planning and works collaboratively to identify case management needs.
Assist clients to access community-based programs after discharge by making and coordinating referrals for all aftercare, medical, housing, and other needed ancillary health and human services.
Monitor service delivery and identify barriers to services through regular contact with clients, providers and treatment team.
Serve as liaison between community providers, clients and the POWER staff including coordination of care with POWER’s contracted Primary Care provider as needed.
Assist clients with transportation and childcare needs to allow for their continued care after discharge from program.
Provide aftercare, outreach, engagement and follow-up activities to clients as needed.
Represent POWER at community meetings, service fairs and other outreach activities.
Maintain and update agency resource directory.
Facilitate psycho-education and other life skills groups.
Administer urine drug screens as needed.
Participate in 24-hour on-call rotation as needed.
Participate in assigned team meetings and consultations as requested, as well as staff development and training activities.
Complete all assigned electronic paperwork, accurately and on time, as determined by POWER.
Maintain productivity standard.
Qualifications, Skills and Knowledge Requirements
Bachelor’s degree in Human Services plus one year related experience preferred. Or, Associate’s degree in Human Services with three or more years in a drug and alcohol setting. PCB credentials preferred.
Excellent interpersonal skills and demonstrated ability to effectively engage clients.
Understands and supports a trauma informed system of care
Strong organizational and time-management skills.
Good written and verbal communication skills.
Good computer skills; preference to candidates with experience with EMR.
Personal recovery experience not required, however, knowledge of recovery, support systems and community resources required.
Experience in a women-centered treatment environment or with gender-responsive care a plus.
Value for and ability to deliver excellent customer service.
Self-directed and flexible to meet the needs of administration and management, and ability to manage multiple tasks.
Model and support clients and staff in health and wellness activities and with using healthy coping skills.
Support organization’s mission with a sensitivity of cultural diversity and workplace harmony.
Ability to secure FBI, Act 33 and 34 clearances.
Working Conditions
Ability to work all hours on-site at POWER’s inpatient program.
Ability to work flexible hours to meet the needs of the program and the clients served.
Ability to work the hours required to complete the job.
