Definition

Provide case management and recovery support services to Renewal Treatment Inc., clients in an Inpatient setting. Other responsibilities will include assisting counselors with treatment goals, home plans, furloughs, contractual reporting, and connecting clients to community resources. This position will assist the client with creating and implementing a service plan during their treatment as well as during their aftercare planning. This person will enforce the compliance of program guidelines and assist the clinical supervisor with the functioning of daily activities, following policies and procedures, and maintaining records. This person will work closely with counselors, counselor assistants, and community resources to strengthen the client’s recovery efforts to reduce recidivism.

Eligibility Requirements

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. 

Education and/or Experience

Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services), or other related field and one 1-year experience in counseling or case management in a drug and alcohol treatment setting. Certification preferred. PA Drivers’ License, High Degree of Ethics, and Act 33/34 required.

  1. Responsibility for case management for clients 
    1. Meet with each client to assess and address case management needs. 
    2. Assist in the orientation of reentrants to the Drug and Alcohol program.
    3. Provide supportive crisis intervention to clients using a strength-based model and Recovery Management Model to provide positive change and independent living skills.
    4. Develop and maintain working relationships with work release and re-entry programs as well as other organizations to help make appropriate referrals for clients. The focus should be on the areas of health, mental health, housing, employment, substance abuse recovery, medical, and the criminal justice system.
    5. Keep accurate and detailed case records according to agency contract requirements.
    6. Maintain records in order in secure filing cabinets according to licensing requirements.
    7. Attend PRT meetings, as needed. 
    8. Engage reentrants not receiving case management services and schedule appointments when appropriate.
    9. Participate in necessary disciplinary hearings dealing with reentrant rule violations.
    10. Work closely with counselor and client in preparation of client’s aftercare plan. 
    11. Complete furlough/home plan paperwork in a timely manner and according to contractual requirements. Provide follow up to the client as received.
    12. Assist with transporting reentrants to mandatory/necessary appointments, such as court and medical. 
    13. Coordinate escorting reentrants to various agencies and assist reentrants with obtaining vital information and documents in preparation of transitioning back into the community. 
    14. Coordinate with the medical staff to ensure reentrant prescriptions are refilled as needed.
    15. Coordinate and assist medical staff to ensure reentrant medication audits are completed weekly, ensuring discrepancies are addressed appropriately. 
    16. Maintain constant communication with clinical supervisors, counselors, counselor assistants, medical staff, security, and the Intake department regarding reentrant progress while maintaining HIPAA standards. 
  1. Reporting Requirements and Data Collection- 
    1. Responsible for submitting weekly reports to Clinical Supervisor. 
    2. Responsible for completing and maintaining accurate case management notes and records per agency contract requirements. 
    3. Ensure home plan and furlough paperwork are completed and submitted in a timely manner.
  1. Other related duties and assignments as required.

Communication frequently involves dealing with confidential issues and dealing with multiple personalities; must be creative. Comply with HIPPA regulations. Rotating on-call. 

Team Skills:

The ability to work in a team environment.

Competency Level:

Must possess a high level of creativity, and organizational and management skills. Maintain 40 training hours per year to include DOC Basic Training and required DDAP training adhering to licensing and contractual requirements.

Equipment/Machinery Used:

Proficient in Microsoft Office and become proficient with all required software systems including the FMX system, Valant, Apricot, and Chart Meds. Utilize Renewal, Inc. provided safety devices and has the ability to drive a company vehicle. The individual in this role must be proficient in the administration of naloxone, CPR, First Aid, and AED.

Performance Appraisal:

Conducted mid-year and annually within the set guidelines, by Clinical Supervisor.
