Social Services Senior Case Manager – Downtown Pittsburgh, PA
At Catholic Charities, Diocese of Pittsburgh, every client we see is our most important client.
We’re seeking a compassionate and supportive Senior Case Manager to join our Allegheny County Outreach Team located in Downtown Pittsburgh to provide intensive case management to individuals and families who are homeless or at risk for homelessness. The Senior Case Manager will also support the Program Director with providing leadership oversight of the department.
With your commitment to service to others along with your education and experience in human services you know the importance of building rapport with clients to help them access the services they need. You will establish a partnership to facilitate much-needed relief to members of a vulnerable population, providing case management and helping clients meet important, practical goals. In addition, you have a demonstrated record of leadership with performance of your job responsibilities that will contribute to a collaborative and successful team.

Key Performance Objectives:
  • Provides case management and service planning for assigned clients who are homeless or at risk for homelessness.
  • Assesses requests for service to determine applicant eligibility.
  • Provides on-going casework and supportive services to assigned clients.
  • Provides information and referral services for other community services in order to link clients with appropriate resources.
  • Provides or arranges for needed supportive services to fulfill service plan objectives.
  • Documents all interactions with clients, service coordination information, and goal planning activities in a timely, comprehensive, and accurate manner to ensure compliance with program standards.
  • Develops new approaches to better serve the needs of clients.
  • Supports the Program Director with managing the daily operation of the department as needed.
  • Serves as a point of contact for caseworkers and administrative staff in the absence of the Program Director as needed.
  • Oversees and documents work of assigned interns within the department.
  • Participates in grant writing/fundraising and public relations activities to create a positive image of the agency and a public climate conducive to financial support.
Minimum Qualifications:
  • A bachelor’s degree in social work or a human services field or an equivalent field is required.
  • A minimum of five years of social service experience that includes intensive case management is required.
  • A proven ability to interact constructively with community agencies and resources is required.
  • Excellent problem solving skills are required.
  • Excellent written and oral communication skills are required.
  • A demonstrated record of leadership in the performance of job responsibilities is required.
  • Previous supervisory experience is preferred.
  • The ability to effectively multitask and meet established deadlines is required. 
  • A demonstrated ability to accurately document client notes and discussions in a timely manner is required. 
  • Proficiency with Microsoft Office products is required.
  • Act 33/34 and FBI clearances are required.
