Licensed Practical Nurse (LPN)

Position Duties and Responsibilities:

• Prepare patient electronic health record and other appropriate forms prior to patient’s appointment. Documents pertinent data in medical records.
• Develop/distribute lists of community resources available to patients.
• Establish and maintain relationships with consulting providers, and other frequently utilized healthcare resources to serve as a bridge across the continuum for the patient.
• Coordinate consult, referral, and follow-up tracking process for the practice.
• Coordinate continuity of care with patient to ensure compliance.
• Assists in general maintenance of clinic and patient care equipment; clean equipment, examination tables, and work areas; stores supplies and equipment, restocks supply carts and communicate supply shortages to government personnel so supplies may be ordered.
• Assist with orientation of new clinic personnel. Participate in education and professional development through continuing education programs.
• Ensure safe handling, storage, and administration of vaccinations.
• Effectively communicate the importance of vaccinations and educate patients on vaccines’ efficacy and safety.
• Review patients’ medical history and be aware of any allergies to ensure a safe vaccination process.
• Provide post vaccination monitoring, including recognition and managing of any physical or emotional reactions to vaccines.
• Monitor vaccines temperature requirements.
• Support the public health directorate by providing mass vaccinations to active duty, civilians, and contractors at various MTF’S and military tenant commands within a 40-mile radius.
• Perform various screenings such as vision, vital signs, spirometry, audiometry, and EKGs, reviews result for completeness and accuracy.
• Verifies and orders all indicated laboratory studies, x-rays, immunizations, and other ancillary tests as appropriate.
• Answers telephones. Schedules appointments and coordinates pre-placement and routine physical exams.
• Maintains logs and tracking systems per departmental requirements. 
• Ensures patient documentation is complete and accurate and performed in a timely manner (per departmental requirements). 

Minimum Qualifications: 

• Licensed Practical/Vocational Nurse Certificate.
• Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
• One year of practical or vocational nurse experience after graduation.
• Current, full, active and unrestricted license as a practical or vocational nurse.

• Must be a US Citizen.
