Title: Licensed Practical Nurse (LPN)

Reports to: Project Manager

Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in Acute Care, Primary Care and Specialty Clinics in Charleston, South Carolina, and VA clinics. The LPN will perform those approved Medical Center procedures that are described in the Licensed Practical Nurse Act, Vocational Nurse VA Qualification Standards. The LPN will manage patient care and demonstrate competence in caring for adult psychiatric, medical, and geriatric patients. Under the supervision of the nurse manager, the LPN works closely with others as needed int the Specialty Clinics and Primary Care setting throughout the Ralph H. Johnson VAMC and VA clinics (CBOC’s). The LPN functions both dependently and independently with guidance and direction from the Nurse Manager/designee.

Objectives of this role

• Demonstrates competence in caring for adult psychiatric, medical, and geriatric patients.

Responsibilities

• Prepares, administers, and appropriately documents actions taken specific to commonly prescribed oral, topical, subcutaneous intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient’s response to medication administered and the reporting of any noted change in patient’s condition to RN or MD/DO.

• Demonstrates ability to serve as a responsible member of the nursing team and interacts appropriately with patients, family members, professionals and other supportive personnel involved int the delivery of patient care, incorporating acceptable, established customer service standards into practice.

• LPN may be utilized as needed in the Acute Care Medical/Surgical Areas, in ambulatory/outpatient care clinics, and outpatient lab.

• LPN may be required to work with COVID 19 positive patients and Patients Under Investigation (PUI), for COVID 19.

• Performs other duties as assigned by Management Staff or Supervisor.

Required skills and qualifications.

• Graduate of a State approved school of practical/vocational nursing.

• Unrestricted LPN license from any State.

• Current Cardiopulmonary Resuscitation (CPR)/Basic life Support (BLS) certificate accredited by the American Heart Association.

• Minimum 2 years current experience and (must have worked within the last six months) in a primary care setting.

• Staff assigned to Charleston VA Primary Care/Specialty Care Clinics unit can expect to work 8 to 10 hour shifts.

• Current 2-step PPD Testing

• Current tetanus Toxoid

• Immunity to Rubeola, Rubella, varicella

• Current Hepatitis B Vaccination

• Current COVID-19 Vaccination

• Current Security Clearance
