Description

JOB SCOPE: The Family Social Worker is responsible for providing services to children, adolescents, and families in a residential setting. Assists the Program Director in various aspects of residential operations to ensure compliance with all regulatory agency requirements, as well as PCMH policies and procedures. Major areas of responsibility include individual and family counseling, crisis intervention, assistance with formulation and achievement of goals and objectives, providing after school activities and groups, entitlement advocacy, and ADL skill training. Ensures quality case management and care for all families, maintaining comprehensive case records, providing leadership and supervision of direct care staff, and responding to crises. 

ESSENTIAL FUNCTIONS:

  • Maintains case records in accordance with regulatory requirements; reviews case records and signs off as needed; approves admission and discharge summaries.
  • Ensures that family head of household follows appropriate psychiatric and medical plans; approves individual service plans. 
  • Orient new family admissions and complete intake admission documents.
  • Provides assistance to resident families around all Residential Counselor activities, as needed.
  • Assist Program Director in providing ongoing supervision to Family Residential Counselor and Front Desk Counselors, as needed.
  • Maintains a resident caseload, as needed.
  • Conducts and participate in case conferences.
  • Assists in scheduling and planning recreational and social activities, specifically focused on children and adolescents.
  • Facilitates groups in the residence.
  • Reports all untoward incidents in accordance with internal and regulatory procedures and completes follow-up on incidents.
  • Prepare written correspondence and notifications on resident related issues.
  • Conducts internal audits of clinical case records and all other program documentation to ensure compliance with city, state, and federal regulations. 
  • Monitors compliance with all applicable safety and health code requirements.
  • Maintains productive working relationships with provider groups and referral sources.
  • Enforces terms and conditions outlined for residents in all Lease and Lease Riders.
  • Where applicable, helps to ensure compliance with LIHTC regulatory requirements, including working with residents to obtain required documentation.
  • Participates in special projects assigned by senior management.
  • Performs other duties, consistent with the goals and objectives of the program, as assigned by the Program Director, Regional Director, or Vice President of Residential Services.

Scheduled Shift: Tuesday- Saturday12pm to 8pm

Rate of Pay: $30.85 Hourly

EDUCATION AND EXPERIENCE:

  • Master’s Degree in a Mental Health discipline or related field.
  • Computer literate with knowledge of Microsoft Office, spreadsheets, and publishing software.
