Description
JOB SCOPE: Within a rehabilitation and recovery orientation, will provide group and individual counseling as well as case management services to clients with serious mental illness. Will work on a multi-disciplinary team (s) to provide a full range of program services, including skills acquisition.
ESSENTIAL FUNCTIONS: Responsibilities include, but are not limited to the following:

• Assist with the oversight of PROS in conjunction with Director / Clinical Coordinator/PROS Supervisor
• Ensure the timely submission of reports including, intake documents, assessments, safety plans and others.
• Provide intake assessment and individual recovery planning to eligible consumers.
• Provides individual and group counseling and case management services to clients.
• Concurrent documentation
• Carry a caseload of clients as determined.
• Maintain case records including completing progress notes.
• Ensures all relevant case related information is inputted into case files.
• Coordinate service delivery for clients.
• Lead or co-leads groups.
• Provide and/or assure that each case assigned receives regular contact as needed.
• Provide case management services.
• Create program linkages for participants support services.
• Attend and participate in program meetings and case conferences.
• Provide coverage in other staff's absence.
• Participate in individual and group supervision.
• Perform other duties as assigned by PROS Director/PROS supervisor.

Schedule Shift: Monday-Friday 9am to 5pm
Salary: $60,000 Yealy
Qualifications
KNOWLEDGE:

• Bilingual Spanish/English preferred

EDUCATION AND EXPERIENCE:

• LMSW/LMHC in social work preferred.
• Master's degree in Mental Health discipline or related field.
• Minimum one (1) year experience working in a social service agency, rehabilitation program, psychiatric residence, or similar setting.
