Job Summary:
This position is responsible for developing and monitoring the case plans of individuals in the SOAR (Skills, Opportunity, Achievement, Responsibility) re-entry program by performing the following duties.

Job Summary:
This position is responsible for developing and monitoring the case plans of individuals in the SOAR (Skills, Opportunity, Achievement, Responsibility) re-entry program by performing the following duties.
Essential Duties:
- Coordinates, collaborates with, tracks, and monitors participants to ensure compliance with their case plans.
- Works closely with partner agencies to ensure program participants are engaged and supported in services, including employment/training, educational attainment, and other supportive services.
- Collaborates with other community agencies as needed on an individual participant need basis.
Job Duties:
- Services a caseload of 25-30 active participants, all of whom will be justice-involved adults.
- Provides individual case management sessions with all participants at regularly scheduled intervals.
- Conducts initial screening to determine the level of service and support needed.
- Develops individual care plans for all clients that identify barriers to successful reentry.
- Documents all client contact and progress, including time and date, type of contact, outcome, and plan of action.
- Provides or broker services to address participants’ needs holistically.
- Provides regular updates to the Program Manager.
- Attends program and agency-related meetings and conferences.
- Works closely with the program funder in the collection of data, report writing, and follow-up.
- Maintains punctual, regular, and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Respectfully takes direction from manager.
- Other duties as assigned.

Experience/Education:
- Bachelor’s degree in social work, education, criminal justice, psychology, or related human services field with two years of experience providing counseling/case management services or equivalent combination of education, training, and relevant/applicable experience.
- Experience in outreach, crisis intervention, and conflict resolution skills.
- Experience working with communities of color, particularly African American/Black communities.
- Thorough knowledge of the theories, methods, and techniques of social casework, counseling, motivational and strength-based interviewing, and cognitive behavioral techniques.
