The Equi Institute is a nonprofit organization based in Portland, Oregon. Our mission is to enrich the health of the Trans, Queer, Gender Diverse, and Intersex communities through trauma-informed care, culturally affirming services, and social justice advocacy.
Equi’s Community Health Program is designed to connect unhoused or insecurely housed LGBTQAI2S folks - including the intersections of BIPOC, disability, elders, and people who use drugs - with traditional health workers to facilitate better connection with health care, peer support, wraparound services, harm reduction, basic first aid, wound care, and COVID-19 prevention. Our goal is to bring social services, and mental and physical health care to the people who need it most.

Responsibilities:
- Support the peer worker team in documentation, goal mapping, and case coordination.
- Oversee case management data collection and ensure accuracy and completeness.
- Assist the intake specialist in refining data collection processes to enhance efficiency and effectiveness.
- Balance case loads effectively to ensure equitable distribution and optimal support for participants.
- Serve as the HMIS (Homeless Management Information System) lead for the team, ensuring compliance with data entry and reporting requirements.
- Conduct comprehensive participant assessments to identify needs, strengths, and goals.
- Develop and implement individualized care plans in collaboration with clients, addressing housing, healthcare, employment, and other social determinants of health.
- Provide ongoing support and advocacy to participants, connecting them with resources and services to address their needs.
- Maintain accurate and up-to-date participant records, including progress notes, assessments, and service plans.
- Collaborate with internal and external stakeholders, including social service agencies, healthcare providers, and community organizations, to coordinate services and support for participants.
- Monitor participant progress and outcomes, adjusting care plans as needed to ensure optimal outcomes.
- Provide trauma-informed crisis intervention and support as needed during day center hours

- Physical Demands: While performing the duties of this job, the employee is regularly required to walk, sit, use hands, and talk or hear. The employee is occasionally required to stand, and reach with hands and arms.
Education/Experience:
- High school diploma or general education degree (GED); and two to four years related experience and/or training; or equivalent combination of education and experience.
- Bilingual preferred
- Bachelor's degree in a relevant field (e.g., social work, psychology, public health), high preference for LCSW license
- Minimum of 2-3 years of experience in case management or a related field, preferably in a nonprofit setting.
- 1-2 years experience in grassroots community organizing, campaigns, or direct service work
- 1-2 years experience working in healthcare and/or mental health care
- Experience working in a social services or community-based organization setting.
- Knowledge of trauma-informed care, anti-oppressive practices, and person-centered approaches.
- Familiarity with HMIS and data collection processes preferred.
- Experience working with complex mental health needs
- Resident of Oregon and strong ties to county in which you are applying
Certifications & Licensure
- LCSW
- Valid driver’s license
Work Schedule and Location
The work in this position is performed in-person. You will be provided with a computer for your work. We require masking for all in-person services.
