Your Impactful Role

As an RN Case Manager, you will be an integral part of our Bundled Payment Care Initiative, managing a group of Medicare orthopedic patients throughout a ninety-day episode of care. This telephonic case management role involves gathering clinical information, creating comprehensive care plans, and collaborating with various healthcare professionals to ensure a seamless continuum of care.

• Manage a group of Medicare orthopedic patients through the Bundled Payment Program
• Conduct telephonic health assessments to determine risk stratification.
• Document clinical assessments and care plans in Pinpoint case management system
• Educate patients and caregivers on the healthcare delivery system expectations.
• Collaborate with the multidisciplinary team in acute, post-acute, and outpatient settings.
• Follow up with patients at predetermined intervals, both telephonically and on-site
• Evaluate and revise care plans based on patient health status changes.
• Communicate effectively with providers and care team members to facilitate transitions of care.
• Identify and address gaps or barriers in treatment plans.
• Understand and adhere to the Bundled Payment Program design and reporting requirements.

Qualifications

• Current unrestricted State of Georgia RN license
• 2-3 years of nursing experience in a hospital setting or telephonic case management
• BSN certification preferred.
• Experience with discharge planning and outpatient care coordination
• Medicare or Medicare Advantage experience is a plus.
• Orthopedic and geriatric experience is advantageous.
• Proficient computer skills, including MS Excel, PowerPoint, Word, and Athena EMR software.
• Excellent interpersonal and customer service skills
• Ability to work independently.
