We are seeking a highly organized Case Manager to join our team in Atlanta, GA. The successful candidate will be responsible for managing a caseload of clients, coordinating services, and ensuring that clients receive the support they need to achieve their goals.
Responsibilities:
- Manage a caseload of clients and provide ongoing support
- Coordinate services and resources to meet the needs of clients
- Develop and implement individualized service plans
- Monitor client progress and adjust service plans as needed
