Job Summary:
As a Case Manager, you will play a crucial role in ensuring the well-being and quality of life for individuals with developmental disabilities. You will oversee various aspects of the Individual Support Plan (ISP), serve as a contact person, and coordinate essential paperwork and documentation. Your responsibilities also encompass conducting training, monitoring program compliance, and maintaining open communication with consumers, families, and colleagues.
