¨ Perform application process steps, including scheduling orientation appointments and accepting applications.
¨ Explains program procedures, eligibility criteria, waiting list preferences, anticipated waiting times, and other program matters to applicants.
¨ Places applicants on the waiting list in accordance to funding source guidance.
¨ Maintains waiting list, updating applicants’ personal information and waiting list placement, as required.
¨ Communicates with applicants to provide documentation to support their eligibility with the program(s). Obtains necessary verifications and other required information ensuring that all forms are properly signed and filed.
¨ Prepares and manages client case records, to include case notes and pertinent documentation to support decisions and results achieved, in the Agency’s client database for the supervisor’s review, in compliance with Agency and funding source policies and procedures.
¨ Explains program requirements to landlords and rental agents.
¨ Develops and maintains a network of outside referral sources by telephone, written correspondence, and in-person contact. Provides applicants with information for other social service agencies and prepares referrals, as needed, in order to connect applicants to requested services.
¨ Conducts outreach activities designed to ensure a wide range of participation for potential applicants.
¨ Ensures new applicants are processed efficiently to meet required production goals.
¨ Submits monthly reports to DEO through the SERA Grant Management System.
¨ Collects survey feedback for program improvement purposes.
Other Functions:
¨ Answers all inquiry calls and emails from prospective applicants.
¨ Submits requisitions and other supporting documents, in order to execute program services and results.
¨ Performs various general duties, which may include typing and filing using technology systems to achieve program results related to the National Housing Indicators.
¨ Performs other job-related duties as may be assigned.
Required Education and Experience: Applicant must possess a high school diploma or equivalent, plus a minimum of two years’ experience in case management or an acceptable combination of education and experience. Must be proficient with data entry and various computer software applications.
