We are looking for Case Managers to join our team, which is responsible for the coordination and organization of services to our clients. Work with residents to develop person-centered, strength-based care plans using a Housing First approach, and in the context of an Integrated Primary and Behavioral Health Team model. Act as a positive role model by partnering with residents to develop activities and services which lead to positive outcomes including improved health, wellness, and stability.
As a Case Manager you will:
1. Assist residents in identifying skills and develop positive growth. Create a plan to achieve their goals.
2. Identify with each resident the steps that lead to work and create ways to motivate the tenant to complete these steps.
3. Work with blended management team to ensure building standards are met.
4. With blended management team members, design and implement daily activities that promote recovery, employment and personal growth.
5. Advocate as necessary on the behalf of the residents (public aid, social security, home services, etc.)
6. Develop tenant leadership and involvement in civic life through programs such as neighborhood clubs and organizations
7. Perform interviews and reference checks for applicants and make recommendations for tenancy.
8. Participate in quality efforts to promote organizational development.
9. Keep proper records on case management and/or other activities
Education: Bachelor’s degree in Social Work or related field required.
Experience: 1 year of experience in a human services or related field preferred.
Licensure: Licensed Social Worker or Licensed Professional Clinical Counselor preferred. Valid driver’s license.
