Community Case Manager in Columbus, OH:

Working Conditions:
Busy community-based work environment with regular contact with a diverse group of people who are in crisis.

  • Full-time position
  • Monday-Friday, 1st shift
  • Pay: $21-$28/hour depending on experience

General Summary:

Responsible for providing customized community-based support and services to adult survivors of Intimate Partner Violence/Domestic Violence (IPV/DV) who exit shelter into their own housing and individuals who can remain safely in the community. Includes assessment of strengths and needs, development of action-oriented goal plans and coordination of referral and linkage resources to assist clients in successfully and quickly addressing their challenges and needs. Use a trauma-informed approach as well as other case management best practices with a primary focus on safety and maintaining housing.

Requirements:

  • Minimum two (2) years working in a social service setting required
  • Experience with Crisis Intervention, Case Management and Advocacy
  • Commitment and ability to work as a team with diverse staff, volunteers and clients; using a Trauma-informed Care, approach, preferred
  • Valid Ohio driver’s license, access to reliable insured transportation and ability to meet LSS insurance standards required

Education:

  • Bachelor degree in social work or related field preferred or equivalent experience required. LSW preferred.

Required Knowledge, Skills and Abilities:

  • Knowledge of Domestic Violence, community resources and ability to engage clients experiencing trauma
  • Sensitivity to the needs of agency clients and ability to maintain confidentiality
  • Strong oral and written skills
  • Knowledge of human service delivery systems and systems advocacy
  • Ability to work independently and as part of team to deliver services
  • Knowledge of basic computer skills

Essential Duties and Responsibilities:

  • Receive referrals from hotline resource specialist (HRS) and shelter case management staff and conduct initial interviews with potential clients.
  • Assist residents, case management staff and community partners with securing and preparing their housing for occupancy as quickly as possible.
  • Work closely with clients to develop and implement intentional action-oriented goal
  • plans based on the CTI model, with a focus on maintaining housing, enhancing safety, and creating a meaningful quality of life.
  • Implement planning strategies that reflect client strengths and needs, safety of the household, client responsibilities, and action steps to help clients in achieving their goals.
  • Provide budgeting and other planning assistance toward the goal of housing retention and safety.
  • Refer clients to partner agencies and community resources that benefit their short and long-term goals including physical and behavioral health care needs.

Job Type: Full-time

Pay: $21.00 - $28.00 per hour
