Catholic Social Services is Hiring a Case Manager Near Columbus, OH
- Coordinates with Program Director and Supportive Services Social Workers in developing and carrying out service plan goals.
- Maintain and balance a caseload of 20-30 clients.
- Meet annual productivity expectation of 1074 hours or approximately 89.5 hours monthly.
- Complete home visits and telephone calls to assist with case management functions. Assist in accessing community resources, including making referrals and/or completion of benefit applications or other required documentation.
- Accompanies clients to medical appointments; communicates with medical personnel at the direction of the client.
- Assists clients in maintaining necessary benefits and health insurances include coordinating appointments, completion of benefits applications or obtaining required documentation and/or attending appointments.
- Enter all case notes within two business days of client interaction.
- Provides evidence of referrals given or made to community support, resources, and other services providers in case notes service fields within two business days.
- Represents the agency with the community and follows office procedures.
- Associate degree in human services, Education, or related field, required or bachelor's degree, preferred.
- Two (2) years of case management service experience with diverse, low-income populations, or combination of education and experience.
- Knowledge of community resources and entitlement programs.
- Valid Ohio Driver’s license and state minimum car insurance coverage.
- Ability to meet with clients in the community.
- Excellent organizational skills and attention to detail.
- Excellent written and verbal communication skills.
- Pass a BCI/FBI criminal background check and drug screening.
- Proficient in Microsoft Office Suite or similar software.
- Typical office environment, with minimal exposure to excessive noise or adverse environmental issues.
- In the community as needed for essential responsibilities.
- Prolonged periods sitting at a desk and working on a computer.
- Ability to lift 15 lbs.
