Hope 4 2Morrow Counseling & Treatment Center is Hiring a Case Manager (FT) Near Columbus, OH

Hope 4 2morrow is looking for a full time Case Manager
Responsibilities: ALL positions provide clinical and/or therapeutic services to youth with identified mental health and/or substance use diagnosis.
Case Manager: 
  • Hourly Position pay range is $21-$23 per hr. depending on education and experience. 
  • 8 hour shifts, Monday thru Friday (May include some weekends in times of crisis). 
  • Medical, Dental, Vision, 401(k), PTO, Employee Assistance Program. 
Qualifications: 
  • Bachelor's degree in Social Work, related field, or minimum of two years college credit plus three to five years of work experience in the field of Human Services or QMHS training adequate to perform job duties according to Ohio Law. Will consider a combination of education and experience. 
  • QMHS/CDCA Certification/License or higher preferred, but not required. 
  • Experience working with children and adolescents with behavioral health or substance abuse issues preferred. 
  • Interact in a pleasant and professional manner with other employees, physicians, providers, clients, and client's families to promote a team effort through cooperation and respect for fellow employees and those we serve. 
  • Organizational and critical thinking skills, writing, speaking clearly, answering telephone, fine dexterity, handling and gripping. 
  • Carrying/lifting/transferring (min 50 lb.), communicating, stooping, kneeling, squatting, reaching, standing, climbing, walking, and repetitive activity. 
  • Calculating, interpreting information, analyzing, assessing, evaluating, explaining, teaching, attention to detail, memory and reasoning. 
  • Satisfactory drug screen and criminal background check. 
Duties include, but are not limited to: 
  • Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients' needs are met. 
  • Insure communication between all components of treatment. 
  • Attend court hearings as deemed appropriate. 
  • Ensure documentation occurs according to policy and State compliance. 
  • Attend treatment staffing with prepared report that accurately represents client's response to treatment. 
  • Facilitate individual and group therapy or other activities as assigned. 
  • Function on agency quality assurance or other committees as assigned. 
  • Attend supervisory sessions with prepared agenda. 
  • Ongoing assessment of client needs and document in ISP/ITP. 
  • Assist youth in achieving personal independence and managing basic needs as identified by the client and/or parent or guardian and/or the Director. 
  • Facilitation of further development of daily living skills, as identified by the client and/or parent or guardian and/or the Director. 
  • Coordination of the ISP/ITP, including services identified in the ISP. 
  • Assistance with accessing natural support systems in the community. 
  • Linkages to formal community services and and other systems. 
  • Coordination and assistance in crisis management and stabilization. 
  • Assistance as appropriate to the care of clients, education and training specific to the individual needs, abilities and readiness to learn. 
  • Facilitate Mental Health or AOD interventions that address symptoms, behaviors, thought processes, etc. Assist youth in eliminating barriers to seeking or maintaining education and positive family and social interactions. 
  • Activities that increase the client's capacity to positively impact the client's environment. 
  • Adequate knowledge and ability to navigate EHR (Electronic Health Record) system and/or other system required. 
  • Comply with all State regulatory requirements, agency policies, in-service, and all continuing education training as required by the Director and/or the agency. 
  • Assume additional duties as assigned by the Director or Supervisor. 
ALL positions require a compassionate individual who is dedicated to providing quality mental health care. The ability to work effectively with diverse populations and utilize various therapeutic modalities is essential. Strong communication, organizational, critical thinking and interpersonal skills are also important for success in these role.
