Travel Offer:

JOB DESCRIPTION

• Shift: Monday - Friday (8am-5pm)
• Weekends: Some weekend coverage is required
• Call: Some call may be scheduled, but only on an emergency basis

Requirements:

• Must have OASIS (Outcome and Assessment Information Set) Experience
• Home Health and Hospice Experience - Required
• Experience traveling from patient to patient for home visits
• Must be able to manage and adjust to changes in schedule
• Must have driver's license in good standing
• EMR system used is HealthWyse (Careficient)
• Outpatient care

CERTIFICATION REQUIREMENTS

• BLS (AHA)
