Home Health LPN :
We are seeking a skilled and compassionate Licensed Practical Nurse to join our healthcare team. As an LPN, you will play a crucial role in providing quality patient care and assisting with various medical procedures. Shift: Monday - Friday (8am-5pm)

Weekends: Some weekend coverage is required

Call: Some call may be scheduled, but only on an emergency basis

Requirements:

-Must have OASIS (Outcome and Assessment Information Set) Experience

-Home Health and Hospice Experience required

-Experience traveling from patient to patient for home visits

-Must be able to manage and adjust to changes in schedule

-Must have drivers license in good standing

-BLS required

-EMR system used is HealthWyse (Careficient)

-Outpatient care

-Facility does their own Background Check as well

Responsibilities
- Provide skilled nursing care to patients in their homes, adhering to BLS Certification standards.
- Conduct patient assessments and develop individualized care plans.
- Administer catheterization and tube feeding as required.
- Utilize EMR and EHR systems to maintain accurate patient records.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Assist with IV infusion and medication administration.
- Implement patient education and health coaching strategies.
- Ensure compliance with HIPAA regulations and maintain patient confidentiality.
- Work closely with patients receiving hospice care, focusing on comfort and quality of life.
- Utilize physiological knowledge to assess and address patient needs effectively.
- Provide support to patients with developmental disabilities and Alzheimer's care needs.
- Coordinate care with other healthcare professionals to optimize patient outcomes in a home health setting.

Job Type: Contract

Pay: $44.00 per hour

Expected hours: 40 per week
