• Position: Home Health RN/ Hospica

• Location: Newport, VT

• Duration: Contract

Required:

• · Certification Required: BLS/CPR/

• · License Required: Active RN license

• · Vaccination Required: Covid Vaccine

Responsibilities and Duties:

• -Must have OASIS (Outcome and Assessment Information Set) Experience
• -Home Health and Hospice Experience required
• -Experience traveling from patient to patient for home visits
• -Must be able to manage and adjust to changes in schedule
• -Must have drivers license in good standing
• -BLS required
• -EMR system used is HealthWyse (Careficient)
• -Outpatient care
• -Facility does their own Background Check as well
