ShiftType / Shift Times / Hours / Day : Nights 8

Shift Notes : 8hr Nights 40hrguarantee

Weekend Requirements : Every other weekend / Holiday coverage required

OnCall Requirements : None

COVID Requirement : COVIDVaccination Series and Booster Required for this need. Exemptionsare not accepted by this client.

Traveler Policy : Thefacility will not take local travelers they must live 50 miles awayfrom the facility.

Requested time off during this assignment MUSTbe noted and verified at upload. RTO REQUESTS AFTER UPLOADWILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILLBE ACCEPTED (less is better).

Traveler must be willing towork ChristmasNew Year Day Block (If Needed / Scheduled). RTO duringthese holidays will not be approved.

Observe each residentcontinuously alert to changes in mental and physical wellbeing;notify the Supervisor of pertinent observations and discuss theneed to notify the physician;

and receive Resident Status reportsfrom offgoing shift and give oncoming shift a detailed report ofeach residents status and events of the shift.

  • Accurately receive transcribe and carry out physiciansorders.
  • Administer medications and treatmentsas ordered by the physician and make proper documentation via acomputerized system of MARs / TARs and electronic documentation.

Order and maintain all pharmaceuticals for the units.

Supervise LNAs following licensing requirements under theNew Hampshire Board of Nursing. Assign and measure nursing staffresident care consistent with abilities job description and scopeof practice and supervise the care given;

and assist directcarestaff with nursing care as necessary and as time allows.

  • Accompany physicians on resident rounds on their assignedunit(s) to address residents medical needs and to facilitatecommunication during the 60day review.
  • Administer the provision of oxygen following facilityguidelines and procedures. Administer gastrostomy / jejunostomyfeedings following policy and procedure.

Perform wound care andknow wound vacs.

Perform regular duties on theunit such as checking appointments for the day; checking PhysicalOccupational and Speech Therapy appointments;

checking activityboard; reminding staff of ambulation lists weights and bloodpressures and documenting nutritional charts.

Monitor LNAdocumentation and update LNA clipboard; oversight of infectioncontrol practices on the unit; ensure that the unit is clean andhazardfree.

Following the chain of command report any problems tothe Nurse Manager / Nursing Supervisor. Encourage LNA observationsand include them when documenting and reporting.

  • Admit discharge and transfer residents completingdocumentation and notifications as necessary.
  • Complete 24hour report for his / her assigned unit(s);check new orders; complete list of residents on antibioticsprecautions etc.
  • maintain consistent communication with the NurseManager / Nursing Supervisor regarding resident issues or concernswith staff;

differentiating between acute and nonemergencyproblems; and attend all mandatory staff / educational meetings andnonmandatory meetings as assigned.

  • Communicatewith residents families to resolve resident concerns and problemsnotify family members of new orders and obtain consent whennecessary.
  • Receive incoming calls anddetermine the appropriate response required.
  • Assume responsibility for the management of assignedunit(s) during his / her shift under the direction of the NursingSupervisor (NS) or Director of Nursing Services (DON).
  • Follow State and Federal Regulations regarding thestorage usage and documentation of controlled medications includingcounting at the change of shift;

notify the appropriate Supervisorif discrepancies are present.

Assist with theorientation of new nursing employees on their assigned unit(s).Communicate with DON Nurse Manager or SDC regarding the continuingeducation needs of the nursing staff and assist in the delivery ofstaff education and training as needed.

Encourage and assign staffto attend inservice education meetings. Evaluate assigned nursingstaff members according to facility policy and procedure.

Initiateor effectively recommend disciplinary action asnecessary.

  • 3 year CURRENT experience (within 6mos) NONNEGOTIABLE
  • Must bewilling to float
  • IVCERTIFIED PREFERRED
  • Previous travelexperience Highly Preferred
  • Rural / smallfacility experience Highly Preferred
