Responsibilities / Duties:

Provide supervision and guidance to participants while maintaining appropriate staff-to- participant ratios.

Assist in structuring activities that are important to the person and for the person in accordance with each participant’s My Plan outcomes.

Assist with daily living activities, including preparing lunch meals/snacks and assisting with personal hygiene.

Develop a rapport with participants to cultivate a positive, safe and supportive relationship. Document participant progress notes, attendance, and goals on a daily basis.

Transport participants to and from community outings while ensuring their safety and comfort.

Participate in activities and community outings with enthusiasm and encourage the participants to try new things that may be of interest to them.

Teach participants life skills that promote independence such as money management, appropriate behaviors, social skills, building healthy relationships, anger management etc.

Maintain a clean and sanitary work environment by completing all cleaning tasks assigned daily.

All other tasks as deemed necessary by management

Minimum Qualifications and Requirements:

High School Diploma

Must be able to lift a minimum of 50 pounds

Maintain a current driver’s license and good driving record with less than five points

Pass a criminal background check and all exclusion list checks as required by DODD

Pass a drug test
