Description

In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services. Alongside the clinical care, researchers and teachers from Metrocare’sAltshuler Center for Education & Researchare advancing mental health beyond Dallas County while providing critical workforce to the state.

The PASRR Care Coordinator will work with individuals, families and nursing facilitates to provide support for PASRR MI Positive individuals. In addition, information will be gathered, and eligibility established for various mental health services. Frequent travel to nursing facilities and psychiatric hospitals in Dallas County is required for this position. Performs routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care model. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Perform routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care model.
  • Create and/or enhance an existing person/family-centered recovery plan that identifies an individual’s goals, objectives, strengths, and preferred involvement of natural supports as well as coordination with internal and external providers.
  • Initiate and collaborate complete unfirm assessments and then organize and document the plan for services to meeting the individuals identified needs.
  • Provide overall service coordination, referral, linkage, and advocacy to individuals at nursing facilities.
  • Perform duties in concert with other members of an interdisciplinary treatment team.
  • Formulate individual service plan based on assessment findings on all new admission. 100% compliance expected.
  • Develop measurable objectives and goals agreed upon by the consumer.
  • Actively assist in obtaining and modifying goals as needed.
  • Document attempts to involve consumers, service providers, and caregivers unless opposed by the consumer) in the service plan process.
  • Review recovery plans every 6 months for adults, or as clinically indicated.
  • Execute recovery plan to assigned consumers in accordance with authorized services, meeting objective measure set forth by the client.
  • Actively engage and document on all consumers assigned to caseload.
  • Participate in all interdisciplinary team staffing.
  • Present psychosocial assessment findings.
  • Encourage consumers to attend staffing. Update team on progress or lack of progress in reaching agreed upon goals.
  • Provide relevant information that might affect treatment.
  • Assessing consumer needs on a continual basis throughout the course of treatment
  • Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services.
  • Coordination with internal and external providers to ensure a holistic approach to care that considers all areas of an individual’s life, the individual’s wants and goals and all treatment interventions.
  • Documenting services in the electronic health record within 24 hours after services have occurred.
  • Contact with individual in service as indicated by level of care.
  • Perform other duties as assigned.

  • Knowledge of Social Drivers of Health and how to assist an individual in service in accessing necessary services.
  • Clinical skill for evaluating accuracy of clinical screening and providing appropriate clinical intervention.
  • Knowledge of Trauma Informed Recovery Planning, Harm Reduction, Motivational Interviewing and Person-Centered Approaches to Care
  • Interpersonal skills to communicate and encourage collaboration among therapeutic team members.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

Required:

  • Bachelor’s Degree in social work, psychology, criminal justice, or related human services field
  • Minimum of 1 year of experience in behavioral health care, working with individuals with severe persistent mental illness and/or substance use disorders
  • Strong knowledge base of Dallas County community providers and Nursing facilities

OR

  • Master’s degree in psychology, Social Work, or related field.

Preferred:

  • LMHA/LBHA, or Nursing Facility experience

REASONING ABILITY:

  • Ability to carry out oral and/or written instructions.
  • Ability to operate as a team member, yet able to make positive, individual judgments.
  • Ability to assess the needs of consumers.
  • Ability to recognize and report side effects of psychoactive medications.
  • Ability to assess a potential crisis and ensure the delivery of services at the proper level of care.
  • Ability to work collaboratively with co -workers.
  • Ability to work in a high stress environment, take initiative and be creative.
  • Ability to drive a multi-passenger vehicle.
  • Ability to successfully use an automated clinical record keeping system.

COMPUTER SKILLS:

  • Intermediate level on MS Excel, Email, and word processing programs
  • Ability to utilize Internet for resources.

CERTIFICATIONS, LICENSES, TRAINING, REGISTRATIONS:

  • TB Test, CPR, First Aid and SAMA to be provided within 30 days for hire.
  • TAC web-based training
  • Certified Community Behavioral Health Clinic Trainings completed within the first year of hire.
