We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The independent Service Coordinator advocates and represents the desires of the individual in service delivery. The Service Coordinator also provides assistance in problem resolution between the provider and the consumer. The Service Coordinator will also be monitoring to determine if specialized services are provided and that the services are helping the person to achieve their desired outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Performs other duties as assigned
Meeting with developmental disability consumers in nursing facilities, listening carefully to ascertain their needs, and helping them determine what services they require
Making recommendations for and explaining services to consumers, based on their needs
Taking their initial service requests
Entering information into our computer system
Help consumers access services to help them achieve the quality of life they desire
Meeting with consumers monthly to ensure they are getting needed services, to evaluate the effectiveness of the services in meeting needs, and to determine new problems and new interventions
Meeting with nursing facility employees to learn their services, to arrange services for consumers, to advocate for our consumers, and to make sure they are taking care of our consumers.
When a crisis arises, be available to help link and assist consumers to secure services that will enable them to prevent future crises
Be the primary consumer contact, ensuring that satisfaction is the end result.

person-directed planning consistent with DADS Person Directed Planning and Family Directed Planning Guidelines for Individuals with Intellectual and/or Developmental Disability;
permanency planning;
crisis prevention and management, monitoring, assessment, and service planning and coordination;
community support services availability and management; and
advocacy for individuals.
QUALIFICATIONS
EDUCATION AND EXPERIENCE:
A bachelors or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. At least one (1) years of full-time experience working in the field of mental health or other developmental disabilities is preferred.
Bachelors or Masters degree in a social, behavioral, or human service field.
MATHEMATICAL SKILLS:
Basic math skills required.
REASONING ABILITY:
Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.
COMPUTER SKILLS:
Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
Ability to utilize Internet for resources.
CERTIFICATIONS, LICENSES, REGISTRATIONS:
Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
