Summary
Position Summary: The Transitional Care Coordinator is responsible for performing administrative and operational functions required in the Care Management Department to support regulatory requirements and patient throughput activities to ultimately ensure patients receive the treatment and services they require post-discharge and are informed of their rights and choice options during the process. Oversees the administrative operations of the Care Management Department and acts as the administrative liaison between all clinical departments, community resources, and third-party payers/insurance companies in support of the work of the department.
Essential Functions: Referral Process Assists in the referral process by transmitting required patient documentation using web-based software system(s) as requested by the Care Manager and verifies the information is received.
Identifies concerns Notifies the Care Manager of any identified concerns or delays encountered.
Delivers patient notices Delivers patient notification letters as required; Important Message from Medicare, Notification of Observation Status, MOON, and others as identified. Obtains the appropriate signature(s) and ensures the forms are provided as required to the patient Provides clerical support Provides clerical and support functions to the Care Management staff with the goal of promoting positive, collaborative communication, teamwork, and expeditious patient transfers and/or discharges.
Verify information Verifies that patient demographic and physician information is accurate. Communicates necessary corrections to patient registration.
Arrange appointments Arranges post-discharge follow-up appointments as directed by the care manager.
Communication Uses problem solving skills to avoid delays. Coordinates with the Care Manager and other personnel as needed. Communicates effectively.
Qualifications/Requirements
Education/Knowledge: Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field
Experience: 1-3 years
Physical Demands:
Light - Exerts up to 20 lbs. of force occasionally, and/or up to 10 lbs. of force frequently to move objects. Physical demands are in excess of those of sedentary work. Light work usually requires walking or standing to a significant degree.
