Medical Director - Program of All-Inclusive Care for the Elderly (PACE)
Are you a board-certified medical doctor living in or around the Chicago, IL area? Do you have a passion for providing care to elderly and underserved populations? LeaderStat is searching for a true community advocate to serve as a Medical Director of a Program of All-Inclusive Care for Elderly (PACE) initiative.
As the PACE program Medical Director, you would be an essential part of building the program from the ground up. The focus of the PACE program is on prevention, which is especially critical in the Southwest area of Chicago, as much of that population is both elderly and Spanish-speaking. These patients likely have faced a variety of barriers in receiving or understanding medical care previously, so excellent bedside manner, patience, and a clear understanding of providing trauma-informed care are critical.
This permanent position is a combination of administrative duties and hands-on patient care, all while being responsible for overseeing the quality and improvement of the program. Direct medical care will be provided to patients in their homes, at the PACE center, and potentially in other healthcare settings nearby, such as small clinics.
As the PACE program leader, you will focus on promoting healthy lifestyle choices in neighborhoods that need it most and serve as a liaison between the program and community physicians/service providers.
Ultimately, the Medical Director will be a trusted resource for elderly Chicago residents, while assuring that safe, quality care is provided to these patients and their families.
